We believe you.
It is the mission of the Commission to transform systems. To make change happen, we need to hear from the people who use these systems every day.
We are happy to talk with you in person or by phone if you prefer (instead of filling out this form, or in addition to filling out this form). Please either check “I would like a follow-up meeting with Commission staff”, or contact us by email or phone at 360-312-5700 x1241 or contact@dvcommission.org.

What should I share?
You can share whatever you'd like us to know about what happened, what helped in your healing, or what it was like to seek support.
Have you reported sexual assault or domestic violence to the police? Did you go to the hospital for a rape kit or injuries from abuse? Have you seen an advocate? Did you talk to a teacher, pastor, or someone else about what happened?
What was that like? Did you feel supported? Could things have gone better?
Or maybe you’ve never told anyone before. Maybe if something were different, it would have felt safe to reach out. We want to hear about that, too, because your voice matters. And your story can make a difference.
We only want to know what’s important to you. Each question is optional. You can share as much or as little as you want.
Who will read my story?
Only Commission staff will receive your message. Any identifying information will be kept completely confidential.
You are in charge of your story. We will not share any part of your story without your consent (see question 8). If you agree to share your story, excerpts may appear in reports (which are published online) or shared at meetings (which are open to the general public). We will not share names or other identifying details. We may edit or redact information to ensure your confidentiality.
You can request to talk to a Commission staff member about how or when we might share your story.
What will happen?
Survivor stories are the center of our work. Your story can teach us what’s going well and what needs to change.
We rely on survivor voices to help us develop policies and best practices, train providers, and educate elected officials.
The Commission cannot resolve individual cases or complaints. We cannot promise that all recommendations will be implemented.
For more information, you can read our PDF document survivor input operational guidelines here.
